Information and Instructions: California Geotechnical Engineer Application
WARNING: If an applicant is unable to complete or print an online form, he or she is NOT relieved of the requirement to meet the final filing date requirement. WE STRONGLY URGE ALL CANDIDATES PLANNING TO USE THE WEBSITE FORMS TO COMPLETE THEIR ONLINE "TYPING" AT LEAST TWO WEEKS BEFORE THE FINAL FILING DATE. We also strongly recommend that you print one or two extra blank forms in case computer or printer problems force you to use a typewriter to meet the filing deadline. The Board is not responsible for any failure by an applicant to meet filing deadlines.
It is the applicant's responsibility to compile and submit a correct and complete application package. A partial application packet will not be considered for evaluation and/or examination. A complete application package must contain the application, application fee, sealed college transcripts (if applicable), and Engagement Record and Reference Forms. (Each Engagement Record and Reference must be in a sealed envelope containing Part A and Part B of the Engagement Record and Reference Form. Your references must place their seal/wet-stamp over the sealed flap of the envelope. You may provide your own envelopes. Do NOT open the envelopes -- they must be sealed to be considered.

Complete and sign the application. Make a copy to keep for yourself.
Submit a check or money order made payable to the "Department of Consumer Affairs."
A minimum of four signed and sealed/stamped envelopes from your references containing Part A and Part B of the Engagement Record and Reference Form. Your references must place their seal/wet-stamp over the sealed flap of the envelope. You may provide the envelopes. Do NOT open the envelopes -- they must be sealed to be considered.
College transcripts IF you are claiming education credit. Envelopes from the college must be unopened.
One self-addressed, stamped post card, if desired.
Certified copies of court documents for any criminal convictions required to be disclosed in response to Question 9 on the application. If you are unable to obtain certified copies of the court documents, you must provide the name/location of the court, the case number (if known), and the date of the conviction, along with a description of the criminal conviction, in the Remarks section of the application.
